We offer same-day results for all evaluations.
DUI Evaluation Documentation:
Driving record from DMV (Court purpose, NOT online free one) from any state you had a driver's license in. If the DUI is in Illinois a copy of the (Law Enforcement Sworn Report). If the DUI is out of state a copy of the POLICE REPORT.
*IF THE DUI IS OVER 10 YEARS WILL JUST NEED THE DATE OF ARREST AND DRIVING RECORD*
Investigative Evaluation Documentation:
Copy of criminal history (Record from any county/State you have a charge in). *NEED TO KNOW DATE OF MOST RECENT OFFENSE*
SAP Evaluation Documentation:
Need copy of failed drug test results, supervisor or safety managers name/address/phone number/email, and must register in the Clearinghouse. *CDL drivers MUST request Neela Williams to be their SAP.*
